F.I.R (if necessary)
Death Certificate/Postmortem Report (if required)
Copy of NIC
Accident Report (News paper cutting if any)
Certified Photographs of the injury to the worker
X-Ray paltes both A.P& Lateral view including X-Ray Reports
Reciept of payment made to worker
Acceptance Letter
Medical Report with details of injury and period of disablement.
Registered copy of a Memorandum of Agreement
Surveyor's report with its bill (if appointed)
Air Tickets (For Travel Accident claims)
Medical Certificates in case of bodily injury (For Travel Accident Claims)
Loss Voucher
Undertaking of missing documents
